Although our services are delivered by a Health and Care Professions Council (HCPC) Registered Occupational Therapist, the information provided through this website or during our consultations is not intended to replace advice from:

The Future You Life Profile is designed to support future planning by documenting your personal wishes, routines, values and preferences.

It is not a legally binding document and does not replace:

  • a Lasting Power of Attorney;

  • an Advance Decision to Refuse Treatment;

  • an Advance Statement;

  • a Will; or

  • any other legal document.

While the document may assist family members and professionals in understanding your preferences, Future You cannot guarantee that those preferences will be followed by third parties.

5. CHC & Complex Care Support

Our CHC & Complex Care Support service provides guidance and preparation to help individuals understand and navigate the Continuing Healthcare (CHC) assessment process.

Future You cannot guarantee:

  • eligibility for NHS Continuing Healthcare;

  • funding approval;

  • the outcome of assessments;

  • decisions made by NHS organisations, Local Authorities or other agencies.

All funding decisions remain the responsibility of the relevant statutory body.
