What Is a Future You Life Profile?

The Future You Life Profile is a professionally written, personalised document that captures who you are, how you live, what matters to you and how you would want to be supported if illness, disability or changes in capacity made it harder for you to explain this yourself.

Created with you by a qualified Occupational Therapist, it brings together your routines, relationships, communication preferences, values, personal history and wishes for future support into one clear and meaningful document.

It is designed to help family members, carers and professionals understand the person behind the care needs, so that future support can remain personal, respectful and centred around what matters most to you.
